Sustainable Pathways for Healthcare Investment in Housing: Lessons from HUB Models

Affordable housing providers are increasingly recognized as critical partners in improving health outcomes for residents. However, securing sustainable funding for resident services, especially those already delivered through resident service coordination, remains a challenge.

First in a series, this case study by Stewards of Affordable Housing for the Future (SAHF) highlights HUB models as a promising pathway for funding resident services such as housing stabilization, benefits access and care coordination, while reducing the need to navigate complex healthcare systems for reimbursement of these services.

HUB models function by centralizing key administrative processes, such as contracting, billing, compliance, and payment, through an intermediary organization. This approach reduces administrative burden. While HUB models can help offset the cost of existing resident services, their success depends on clearly defined service definitions, aligned payment structures, and the ability to integrate documentation and reporting into housing operations.

This case study includes two examples from leading affordable housing providers - Mercy Housing and The Community Builders - demonstrating how organizations are leveraging these models to establish partnerships with healthcare payers and help support resident services delivery. These examples illustrate the practical application and potential of HUB models as a pathway for advancing health and housing partnerships.
